| 二、 | 因應該中心編制及業務異動，該中心提供旨案服務至113年7月31日止結束。 |
| 三、 | 本案服務資訊請貴校(園)公告周知，後續教育輔具等相關服務(如各校身心障礙學生教育輔具評估、專業人員駐點諮詢、輔具調整、實地到校或到宅輔導及維修申請等服務)將移轉至東門國小(本市國小特殊教育資源中心)辦理預約教育輔具駐點服務與諮詢事宜，如有疑問請洽東門國小(本市國小特殊教育資源中心)謝老師，聯絡電話：(03)3394572分機836。 |
